• A copy of valid passport with at least 1- year validity
• Recent 2×2 picture – 2 pieces
• Unified Multi-Purpose ID (UMID)
• Copy of Taiwan ARC or NHI Card (for ex-Taiwan workers)
• Certificate of POEA online PEOS (register at peos.poea.gov.ph)
