Documents Need to be Submitted

• A copy of Passport which is valid for a minimum of one (1) year
• United Multi-Purpose ID or UMID
• Birth Certificate from PSA (Philippine Statistics Authority, formerly NSO)
• For ex-Taiwan workers, submit a copy of Taiwan ARC or NHI Card
• Recent 2×2 photo- 2 pieces
• Certificate of POEA online PEOS (register at peos.poea.gov.ph)

Tentative schedule of interview: June 29-30, 2018 (Friday-Saturday)
