- Passport copy which is valid for at least 1 year
- Employment Certificates
- For ex Taiwan, a copy of Taiwan ARC or NHI Card
- Unified Multipurpose ID (UMID)
- Certificate of POEA online PEOS (log on to peos.poea.gov.ph to get the certificate)
Submit personally the above requirements at the Manpower Registry Division, Windows S or T. Ground Floor, Blas F. Ople Bldg. (formerly POEA Bldg.), Ortigas Avenue corner EDSA, Mandaluyong City.
