- A copy of Passport with at least 2 years validity
- UMID or Unified Multi-Purpose ID
- PSA Birth Certificate (formerly NSO)
- For ex-Taiwan workers, include a copy of Taiwan ARC or NHI Card
- Employment certificates (if any)
- 8pcs 2×2 recent ID photos
- Certificate of POEA online PEOS (register at peos.poea.gov.ph)
- Training Certificate (if any)
